As for the play area that's probably a bit more of a very area, play areas will always be risky of a child falls. The stepping stones will have been assessed as a risk, it's a question of whether the outcome was that children be supervised or actually accompanied on them... If it's the former I'd suspect they may be revisiting that, if it's the latter it sounds like they weren't following their own risk assessment.
