Not sure that there is that much you can do when they really go off on one but

most of the time they know entirely what they are doing and its nearly always about attention. We had a spell where we just picked up our youngest and put him in his push chair (no eye contact and minimal words) in the hallway then went back and carried on with our business and he seemed to calm down at which point you could go back and let him out and explain to him. Now he's a bit older we've introduced the naughty step (which is probably terribly bad parenting and frowned upon) and now i only have to mention the step and it heads off some tantrums early on. He's a little monkey though and is always in control but just pushing the boundaries - he's not been like some of these kids you see who just completely lose control.

Probably does not help in public but i guess you could always just put them in their pushchair and wheel them off to a quiet spot.
