There have been many examples of students being taught the wrong syllabus. I remember one such case in Norwich last year. It is a possibility. Its also a possibility that the examiner did not mark the paper correctly, or that coursework went missing, or as you say it could have been a genuine fail.
As for the 50s school. If you are talking about the TV programme I saw, those kids were expected to learn everything in a matter of weeks rather than the 2 years 50's children would have got (not to mention all the previous schooling they had) so it is not fair to compare. The exams of the 50s are completely different to the exams of today. In the 50's children were taught to pass 1950's exams, today they are taught to pass current exams. Can't compare the two that simply.
