Regarding the penno...
I wonder if one of the problems was that it was simply so late. Once you get to that stage refs know a teams is absolutely desperate and so they are basically looking out for dives - therefore unless they are totally totally certain they tend to interpret things in that light. Might have been more likely to have in given in the first ten minutes.
