The main cock-up was letting the football special queue onto the platform.

So when the regular 2 carriage Peterborough service arrived across the platform, everyone steamed onto that one in a mad crush. It didn't help that the rumour had been going round that the football special was alcohol free - but I'd seen no evidence of this being the case.
