It implies a linear progression with worsening symptoms. My nephew is non-verbal autistic. He can't (or maybe doesn't want to) talk, and he can sit for 3 hours straight opening and shutting the disc drawer on a DVD player. On the other hand, he is quite affectionate and tactile which is supposedly unusual.
