My guesses (with spoliers for anyone who hasn't seen it)

Shaun won't get back with Smell, that ship has sailed. Shaun's story is about his future, not his past.

Kelly will OD for sure, that's been clearly signposted in the first two episodes. Whether she'll die or not remains to be seen.

I assumed Milky was talking to someone in his family. He does have a very large family - his family community is partly what prompted Combo to attack him in 84.

Lol won't die. My thoughts are that Milky might see the change in Combo too late to stop anything he puts in motion but it's possible that there will be an unintended victim of Milky's revenge - I think Woody is more likely to be caught in the crossfire.
