...ALL forms that need to be completed in the event of a transfer are sent by the FA to the club in a 'registration pack'.

If this is correct, it's very difficult to prove that this is just an honest mistake. Even so, clubs have been penalised heavily in the past for far more honest mistakes - to my mind it's not as above board as purely passing it off as an honest mistake.
