At least that's the current medical thinking. There are almost always underlying causes. In fact the medical world thinks there are always underlying causes, although in a tiny percentage they can't detect exactly what it was.

An acquaintance of mine, Joris (flatmate of girlfriend at the time), dropped dead on a basketball court at the age of 21. Fit as hell. Turned out he had a defect with one of the muscles around his heart, which just snapped as he was making a pass.
