I can only think of three occasions when a manager has left us for another side
Mike Walker, Martin O'Neil and Lambert.
On all other occasions managers have left because they've done a bad job.
When Lambert left, I didn't feel like he'd taken us as far as we could go.
Anyway, on to your question, it could be Culverhouse / Karsa, but it could just be luck. Plenty of managers over the years have enjoyed great success for a short period at one club and then not a lot elsewhere.
He inherited an excellent league one squad, Hoolahan, in league 1?! He did a good job of what he is good at, galvanising the fans, players etc. he achieved that at Ipswich too. The difference here though was he actually had a very good squad to work with. At Ipswich, you can do all the galvanising you want, but ultimately the quality of the squad was poor, same at Villa really - that was a club going down the pan due to under investment in that era, regardless of well Lambert did at building up team spirit etc.. At Norwich he was lucky to have both a great squad and a very good CEO in McNally who was also ruthlessly ambitious.
The reality is probably a combination of all the factors, rather than one in particular.
