Builder recommendations please.

Any suggestions for fine city based builders who can organise the whole lot of:

New kitchen and bathroom.
Moving a door or two.
Removing a chimney breast.
Building some nice fitted cupboards and wardrobes.
Small extension.
Put down some wooden flooring.

Any recommendations much appreciated.
