Namely, each missile silo/sub/bomber will be commanded by an officer with a degree of common sense.
If the order to launch comes down from a nutter or kidnapper etc, they are simply going to disobey it.
For a real life example, you have the political officer (?) on the Russian sub during the Cuban missile crisis who overruled his 2 senior officers who had both authorised a launch.
