Printing cd cases

I got some perforated sheets of paper for printing cd inserts but I can't work out how I make/find a suitable template? Anyone got any ideas? It's basically 1 sheet of A4 with a front and back pre perforated. I just want basic characters, nothing fancy btw

Brand of paper is amps and neither the back or website has any useful information
