it is so, so easy to make that kind of mistake, and then there's a terrible sinking feeling! Usually you would ask picture desk to get a pic, they would ask for more info, and you would say 'wales assistant manager'. What happens then is that the caption info on the pic probably says 'Mark Bowen', even if he was there but not in the shot. Job done.

Up against deadline, anyone from the sports editor down would have probably been concentrating on the layout/copy and the eye would skim over the pic.

Yes, it's an embarrassing mistake and there will be meetings with the editor and fingers pointed, you bet! But luckily they are a daily and life goes on
