Trouble is trading is running a lot of Markets operations in complex derivatives and traders get well f**ked off if back & middle office can't keep up; they see them as a drag on the business of making shedloads of money. Risk models are probably generally very good, but risk control lags behind (as trades will be booked in a variety of spreadsheets with only rudimentary tools to bring it all together). And I'd wager that *no* organisation has any idea of their operational risk, although they'll be aware that it exists (but, f**k it, just employ more people to do payments processing ...)
