Almost certainly nothing to do with the SEO company. When I build

sites I ALWAYS discourage the use of contact forms. They have a history of failing - if you are lucky you notice quickly, if you are your friend you don't. Also Wordpress *raises eyes*.

My suspicion would be that the form is a third party wordpress add on that's probably not had any updates for years.
