...i get a good service and my dentist is much more relaxed now because he is in control of his business.

I spent a lot of my adult life abroad and am very used to cost at the point of service. In Japan, health insurance only covered 70% of the cost, so you pay 30% by default but many company's insurance covers an additional 10 or 20%, so I rarely paid more than 10% of the costs.
