If that’s the case then he should have shook her hand when offered, and then

shook the captain’s hand. If somebody offers you a hand, shake it regardless of protocol.

And if he just had a brain fart in that regard (it’s possible when just focused on one thing) then he’s seen her offer her hand so just explain to her afterward.
