Need more protein for breakfast, cut the bread, and do exercise. Note that

without exercise you won’t lose weight (healthily) on any diet.

With exercise, you’ll be more successful if you make sure you have some decent sources of protein, and fewer carbs. Re chocolate if you’re going to have a bar every day make it dark chocolate. Reduce sugar - none of that helps if you’re also drinking a bottle of coke every day.
