(Simple version)
At a company (for example) you can have a direct connection to the internet from your PC, or you can all share a common internet connection by means of connecting not to the internet itself, but to a software service called a proxy (server).
I'd say 99% of companies run 99% of internet traffic through a proxy. Therefore all they need to do is look at the logs on the proxy and they can already see all the sites visited and which accounts visited those sites, and when.
I, for example, have a direct connection to the internet with Firefox and FTP because the proxy does not all FTP access. With Internet Explorer I go through the company proxy in order to access the group intranet sites which only allow access to users going through the proxy.
