Each team gets one referral, and that can only be used to review a decision in the first or last quarters of the pitch.
Use it and be proved wrong and you lose your referral, get the decision over-turned and you keep it to be used again.
Sometimes the VAR can't help, at which point 'no advice is possible' and the on pitch official should stick with their original decision and the appealing team retains their referral.
Most decisions can be reviewed in 10-15 seconds, some take a bit longer.
The on pitch officials are all miked up as well, so you can hear what the teams are asking to be reviewed and the response from the VAR.
