9/10 it's dehydration, but that can have really scary looking effects.

I saw a guy faint during the Sheff half marathon, who then tried to swing punches at the St John Ambulance people attempting to help him, before passing out again. Poor chap was incredibly confused. 30 yards from the finish line too. That was a warm day, but in April, and warm by April standards, nothing like as hot as today.
