Helios flight

cabin failed to pressurise, flight deck oxygen supply was faulty if I recall so the pilots went hypoxic and were incapacitated. A flight attendant accessed the cockpit and tried a mayday call, but didn't know how to change to the correct frequency so no-one heard it.

Plane flew on autopilot until it ran out of fuel and crashed.

All very sad.
