The horrible thing about my aunt's version of it

Is that 95% of the brain is fine. But not being able to convey with your body or speech - basically any motor function - what your brain is thinking. I just can't imagine what that must be like.

A small plus side though, is that the smile and laugh that slowly comes out when a joke is cracked, is brilliant :)
