Hmmmm....I think it's all very worrying. Unconscious on scene

Means the initial insult (primary injury) is severe on its own. The surgery (presumably on a bleed) is to prevent secondary brain injury. Very little can be done about the primary one. GCS 3 pre op is however NOT a good prognostic sign, unless of course he was anaesthetised at the track meaning that's why he was "unconscious".
Time will tell as no real details available.
