Telling insurance company that licence was clean wasn't a great idea. But DVLA won't know that and the insurance company won't know about the points. However I'd strongly recommend telling them about the new ones, then saying something like "so with the ones I already had it tipped me over the 12" or something, feign surprise they didn't know about the old ones "I thought I'd told you, are you sure you haven't got them", some such. If you were to have an accident, not having this all squared away Will Go Badly For Whoeveritis.
Then it sounds like 12 points on totting up so that will mean a ban, default I think is six months.
To get licence back call DVLA and say you've only just realised you never had it back, need it now as unfortunately you've just been done again, they may charge you for a replacement copy or some such. But again get this done as the alternative is not getting it done, which is Not Clever.
None of this is criminal, yet. But get the insurance company sorted out with information and come clean to DVLA.
