Putting the milk is first is such a social faux pas that it has become an adjective in itself. My grandmother had no greater insult for people than to say that they were "a bit milk in first".

I think it originated from people who had cheap chins which might crack if boiling tea was poured in. So they put the milk in first to avoid cracking.
