I'd put something like....

"Please find discussion notes....

blah
blah blah

"'please confirm this is an accurate reflection of what was discussed/agreed'.

then if you get no reply, request confirmation again....

then if you get no reply, send a mail, keeping the entire thread stating IF you dont receive confirmation or a reply within X days, you'll assume they agree with the discussion notes....
