She was supposed to have this assessment for the level of care she needed but never got it

As a result, and because on one occasion she lashed out at a nurse who was trying to take some blood, she was shoved in the nearest 'high-dependancy' care unit, though she was still essentially compos mantis, just suffering from severe dysphasia. She really needed 1-to-1 care because of her mobility issues, but because she never got this assessment, she never get the special assistance. In the end, she fell over when trying to get to the loo and died on the floor. Completely and utterly avoidable.
