Not sure about the rules of added time?

I re-watched the end of the Liverpool game on the official site and they said it was just 3 mins of added time.

The ref finally blew the whistle at 4 mins 38 seconds

WTF?

I thought that refs dont add 30 seconds for subs in added time as they do for normal time.

I cannot remember a game going this far over the added time for a while?
