South America in particular has a real problem with mining safety,

Not to say others don't, New Zealand's record isn't great and neither is South Africa. Unfortunately the companies are now under a lot of pressure to exploit the minerals and short cuts are taken. At least now it's becoming a bit more high profile a lot of the companies are starting to change their ways. It's a shame that lives are still being lost but it really is a dangerous job and these types of incident will happen.
