My suggestion was that every time a goal is scored in questionable circumstances the ref should check with the fourth official. In rugby the ref asks "Is there any reason why I should not award a try?" and the same question could be asked in football - so "no because Crouch was pulling the defender's hair" or "no because there were two Ghanaians offside when he headed it in"
