It's genetic.
I was at a symposium about paleontology a few months back and one of the lecturers was John Hawks (User Posted Link from the University of Wisconsin.
His area of expertise in paleontology is genetics, and during his lecture he focused on and how one of the biggest differences between Homo Sapiens and earlier species was the development rate and path from birth to adulthood. The example gene he used was one the "milk gene" which determines lactose tolerance.
When people call themselves lactose intolerant, they often misuse the term. Milk and dairy products just happen to give them stomach upsets. That's not the same thing as genuine lactose intolerance.
