I have seen a worse miss, though - but it was on telly, and I tried to search for it on youtube and failed to find it.

SiH might be able to help: it was Dutch league, early 90s, the guilty party was wearing stripes (red or blue on white, can't recall) and there was a small or deathly quiet crowd.

Dutch telly MUST have shown it as often as ours have embarrassed Ronny Rosenthal, I'd guess.
