I think its all 4 problems

1. Attitude problem - everyone thinks they are too good and don't need to try

2. Tactical Problem - one word: Worthy

3. A motivational problem - 2 words: Nigel Worthington

4. Unrest in the camp - Francis going, getting relegated, selling lots of players and not getting anyone for big money
