Not all European countries have free at source public health care
Germany has a similar system to Holland (as Steve describes it); employers usually pay the premiums as a negotiated benefit (although when I was still there, they were trying to reduce that). On the other hand, the Danish system is similar to the UK's (without the substantial drain of PFI).
In the US, they simply never had any form of public health care (although they do have MediCare (I think it's called), which is massively inadequate). That's pretty much down to the laissez-faire capitalist model that's been ruthlessly followed since it's foundation
