If you go to their help pages, you'll see a Contact Us button at the bottom right
Otherwise use this: customerservices@premiumtv.co.uk
Also write to the club. I received a fairly encouraging response to my complaint and the more who complain the better. I have since discovered that Neil Doncaster is a Director of FL Interactive who, I think, sold the rights to PTV on behalf of the Football League. I don't know whether that's a good thing or not; it means he has some influence, but also a conflict of interest when handling complaints.
Depending on the response to my latest emails to club & PTV; I'll likely be escalating my complaints to other possibly interested parties, eg. BBC, OfCom, Consumers Association (of which I think I'm still a member).
Then it'll be IT sites such as The Guardian Technology and IT Register.
