Ring up your GP, ask for appointment with the practice nurse.
Go and see the nurse. He/she will look up your destination on a chart, see what jabs you need, give you them that they can, tell you to go to Boots for malaria stuff, possibly send you to a specialist jabs centre for anything tricky they don't stock - that'll just be another GP's practice or possibly a hospicle.
