Suspend a wire from your tap to a poin directly over where you want the hole.
Every time you use the tap water will run down the wire and drip off. Over hundreds of years, a tiny diamond-sharp stalactite will form, eventually growing long enough to puncture the tile using nature's forces. You can then remove the wire, leaving a small but perfect hole.
