It's tested in terms of norms.

Basically, you got to be a freak to 'fail' one. You then have to show this is your normal level. If it is you get an exception.

But it begs the question why you haven't failed one before if you've been tested multiple times.

However, an example of this is Charlie Wegelius, the best British rider. He's been proved to have a naturally high level of haematocrit which they use as an indicator of EPO usage.

Final point, Landis has a contraindication on an A test. The B test has yet to be tested.
