Seems odd to me.... more facts needed
There are a number of systems and controls such as cab warnings (directive control) and automated braking (corrective control) that form part of the safety control system.
Human Factors analysis but be difficult but need to understand 'why' he went through a red light.
Incident investigation is one part of my day job, although not usually on that scale) so I'm always interested in these from a learning perspective.
