That stuff is really tough, but they have to employ people to check what is being
served up in 3rd party content areas.
When I worked for Orange in the Netherlands, one day we had a complaint and only then realised... Don't know if you're old enough to remember the old wallpapers for pre-smartphones in around 2005, but anyway we had a portal on our website with a company who sold them (just in an iFrame so that part was entirely managed by them) and obviously we took a cut of each sale. Their system displayed the wallpaper images in order of popularity.
The number 1 wallpaper proudly displayed on what to most people was the Orange NL website? Osama bin Laden....
That was embarrassing.
