the confusion is caused by people being wilfully dense
or unwilfully in some cases.
It's not elegant drafting by any means but you probably need that additional wording in some form to avoid legitimising a wrongful 4th sub in normal time (ie if a 4th sub did somehow get made in the first 90, it would only be against the rules if the match didn't then go to extra time)
