I've always thought of it as a fairly obvious leftover meal (though was in my pre-NLP, and therefore pre-sourdough, days - I think that sounds like a definite improvement...)
had never occurred to me that most people in Britain don't do this. Actually I still reckon they probably do, and Jay Rayner's being a bit precious.
