Because you're meant to add on 30 seconds per sub, so that's a maximum of 3 minutes. But refs seem to pick and choose which teams that applies to. I've seen countless games were United or a Chelsea will be winner by a goal make a change in the 90 and the full time whistle still goes dead on what was announced previously. The clashes of heads were not that long, and no treatment was reqired. . You can't add time on everytime a man is down or the balls out of play, we'd be playing 15 minutes every week.
