I managed that a couple of years ago
I stepped on a manhole cover leaving work on the way to the car. The cover - a big cast iron bastard about 2 1/2 feet by 1 1/2 feet - tipped up, my foot shot down the hole and, freed of the weight, the lid tipped back like a guillotine.
Fucking.
AGONY.
The worst of it was, I had ended up in such an awkward position that I couldn't reach the lid and my leg wedged it in such a way that I couldn't get out. Woman passing on a bike had to stop and prise the lid away from my leg.
Luckily, I got away with some impressive gouges out of my leg and a bloody painful knee, but I could very easily see how you could bust a leg and I have wondered about the camera pits at Carra before, too.
